A Note from Attendance:

If your child is to be excused early or has been absent, it is very important that a note be sent in by the parent showing the student's name as shown on the Birth Certificate or name they are registered in school.  Nicknames are not to be used as there are too many students for us to keep track of.

We would also like to request that the parent's name is printed clearly and that a telephone number where the parent can be reached during the day is also included.  We have a number of students whose last names are not the same as the custodial parent.  Your child's Homebase number and grade should also be included on all notes.
